Accuro
OMD DM Patients with 3 K030 and NO Q040 in the past year. (can be downloaded from Accuro Definition Query Publisher)
- A query designed to identify patients with either a diagnosis code of 250 or T90 recorded in History of Problems CPP band, or a minimum of three K030A billings within the previous year, and no Q040A billings recorded within the same period.
OSCAR Pro
OMD Advisory Cervical Cancer Screening v5
- List of patients with overdue, ineligible, refused or no pap prevention, sorted by provider. Report considers the ‘Set Next Date’ feature in preventions module.
TELUS PSS
- Identifies patients with a coded diagnosis of 428 recorded in Prob CPP band or HPH CPP band, a text entry containing ‘CHF’, ‘heart failure’, or ‘congestive heart’ of 428 recorded in Prob CPP band or HPH CPP band or billed with a diagnosis code of 428 in claims 3 or more times. The report produced contains columns indicating the frequency of Q050A billing, the date of the last Q050A billing, and the most recent date the CDM CHF Form was completed.
